Failure analysis of a bridge crane shaft

A paper published in « Case Studies in Engineering Failure Analysis » on april 2014.

Failure analysis of a shaft used in a bridge crane has been carried out. The shaft fractured in the keyway with evidence of fatigue. The geometry of the keyway also promoted the initiation crack because the width and height were erroneously designed. It was concluded that all these factors produced fatigue failure.

Failure analysis of conveyor pulley shaft

Via Sciencedirect.com : A failure analysis was performed on the shaft of a conveyor belt drive pulley. The shaft failed after seven days in service. Failure mechanism was high cycle fatigue. Failure occurred due to improper reconditioning of the shaft.

Consumer Product Accidents – Shaft Failure

Close-up view of failed shaft
Metallurgical failure analysis was requested after a company experienced several identical shaft failures. Macro and microscopic examination revealed that metal bending fatigue was to blame for the failures. The metal fatigue was due to an inadequate filet radius at a diameter change in the subject shaft. The inadequate radius was due to a manufacturing error.
